Getting started with a new veterinary practice involves a little paperwork—and we’ve made it as simple as possible. Complete our new client form online before your first visit to share your pet’s basic information, medical history, and contact details. Having this on file in advance helps our team prepare for your appointment and make the most of your time together.
Peanut’s Law acknowledgment
Virginia law requires veterinary facilities to inform clients about the hours when continuous on-site medical care is available and when it is not. Our Peanut’s Law Acknowledgment form provides a clear disclosure of our facility hours and staffing, so you always know what level of monitoring is in place for hospitalized pets outside of regular business hours.

Wellness care consent form
If your pet has an upcoming wellness visit, you can complete your consent form online ahead of time. Submitting this before your appointment allows check-in to go smoothly so we can focus on your pet right away.
Prescription refill request
Need a refill on your pet’s medication? Submit your request online and our team will follow up to confirm availability and arrange pickup. Please allow adequate processing time for refill requests—if your pet is running low on a critical medication, call us directly at (540) 894-4572 to ensure there’s no gap in their care.
